Team Form

 TEAM APPLICATION FORM

• To serve on team you must:
• have already participated on a VdC weekend
• be an active parish member,particpate in a Reunion Group, and attend Ultreyas,
• pray for the leading of God to give you the will and ability to be on team, and,
• fill out a Team Application Form.

 You can send the forms by regular post or e-mail to the Pre-Weekend Coordinator. Please send Team Forms by Christmas.

The Secretariat chooses the Rector and number of team members. The Rector selects his team from the received forms.
